Case 1: the form has real input fields
If boxes highlight when you click them, it is an interactive (AcroForm) PDF. Open it in Fill PDF form, type into each field, and save. Choose "lock after filling" if you do not want the recipient editing your answers.
Case 2: the form only has dotted lines
